Assist-Life Stage Transition

Community Nursing Care

Specialized support for participants with complex health needs. Delivered by qualified nurses, this service includes clinical care such as wound management, medication administration, chronic disease management, and health monitoring

Development Life Skills

Developing and maintaining essential life skills while living in a shared accommodation setting. This service includes assistance with household chores, meal preparation, personal care, and participation in community activities.

Daily Tasks-Shared Living

Empowering participants to build independence and confidence in managing everyday tasks. This service includes training in areas such as personal hygiene, financial management, cooking, household maintenance, communication, and decision-making.

Household Tasks

Support NDIS participants in managing everyday domestic activities, ensuring a safe and comfortable living environment. This service includes assistance with cleaning, laundry, meal preparation, grocery shopping, and other essential chores.

Participate Community

Supports NDIS participants to engage in social, recreational, and cultural activities within their local community. This service helps individuals build connections, develop friendships, and participate in events or programs, fostering a sense of belonging and inclusion.

Support Coordination

Provide specialized interventions to help participants improve their functional skills and overall well-being. This service includes support from allied health professionals such as physiotherapists, occupational therapists, speech therapists, and psychologists.
